Abstract

女性月經前之黃體後期(late luteal phase),可能出現各式身心不適症狀,統稱為經前症候群(premenstrual syndrome, PMS)。通常PMS 症狀輕微,影響不大,若症狀嚴重,影響日常生活之職業社交功能,在美國精神醫學會之精神疾病診斷分類(Diagnostic & Statistic Manual of Mental Disorders, 4th ed)中,則定義為經前不悅症(premenstrual dysphoric disorder, PMDD)。在DSM-IV 中,PMDD 歸屬未分類型憂鬱疾患(Depressive disorder, NOS)。憂鬱症目前已知與大腦功能有關。利用腦部正子掃瞄(PET),研究人員發現憂鬱症患者前額葉的血流量及新陳代謝會局部地降低,且其程度與憂鬱嚴重度成正相關。這些腦部影像異常也會隨著憂鬱症的緩解而改善。因而,使腦部功能受到影響,導致憂鬱症患者的認知,情緒反應和注意力發生障礙。以Medline搜尋,目前並無F-18 FDG正子斷層掃瞄用以檢測PMDD之研究報告。本研究之假設為經前不悅症之腦部神經生理機轉類似憂鬱症,故我們以F-18FDG正子斷層掃瞄探討經前不悅症個案月經前「症狀期」與月經後「緩解期」腦部新陳代謝的差別。經MINI 診斷會談,選取符合DSM-IV 經前不悅症診斷標準之個案15 例為研究個案,並取15 名年紀相配、無精神科診斷之正常個案為對照組。於月經前月經後接受接受腦部MRI 及FDG-PET 斷層掃瞄造影,並接受經前不悅症症狀嚴重度、憂鬱、焦慮、認知功能及整體嚴重度評估,再以統計比較經前不悅症組與正常組腦部葡萄糖代謝的差異,以及兩組經前經後兩時段腦部葡萄糖代謝的差異。依本科經驗,PMDD 收案困難,尚加上正常對照組,所有個案需於經前經後各作兩次全套評估,故研究以兩年為研究期。
