Abstract
本研究主要由長期追蹤研究來發展及比較採用APP或行動計畫之心房顫動患者自我管理模式的成效。預期的影響有：1.建立的心房顫動患者自我照護管理模式，提高心房顫動患者全面照護，以預防中風、心衰竭等嚴重併發症。2.填補研究缺口：運用資訊科技與行動計畫照護方案的研究，及相關心房顫動患者健康識能的研究。3.建置研究工具：翻譯或建置的「心房顫動認知量表」、「心房顫動自我效能量表」可作為研究心房顫動族群的工具。4.健康醫療政策：本研究結果可以作為長照政策參考，考量心房顫動患者長期照護之特殊性亦可作為未來提升心臟病患者健康識能策略之參考。
