Abstract

The incidence and prevalence of dialysis in Taiwan is still high in the world. Although end-stage renal disease (ESRD) can be maintained through hemodialysis, peritoneal dialysis, or kidney transplant to maintain physiological function and life, but after the doctor told to wash the kidney, equivalent to the death of the alarm sounded, was forced to face a predictable death. Kidney washing is a life course. The quality of life of patients is an important consideration in the view of "patient-centered" care in contemporary medical care. Patients with self-washing kidney began to face diet, work, medical care, marriage and family multiple adjustment; and physiological differences in gender and social and cultural values, resulting in male and female patients in the treatment and maintenance of quality of life, has the same situation and differentiated needs and expectations. For example, patients are also exposed to changes in the appearance of disease (eg, disguising arm-dip hemodilution and worrying face problems), but male patients are less likely to have sexual concerns; female patients are not encouraged to take risks. These, are involved in medical communication and medical sex. December 18, 2015, "Patient Autonomy Act," the bill read the third time, but Taiwan's dying culture for pre-doctorate, the concept of patient autonomy is weak, kinship-related decision-making involved in deep. The current clinician for the condition of end-stage renal disease informed, consider the end of the communication strategy is how to wash the kidney, the patient's needs and expectations is how? The purpose of this study was to (1)analyze the patient's needs by gender differentiation in hemodialysis patients, as well as the gendered problems encountered in the medical department, through (2) the development of genderaware clinical Lesson plans (including disease condition notification and communication, dying and suspended dialysis), to implement the medical sexfriendly and gender justice.
