Abstract

類胰素生長因子(IGF-I and IGF-II)是具有促生長作用之小分子蛋白質(分子量為7500Dalton),其最主要之功用是為生長激素(growth hormone; GH)的第二訊使(secondmessenger)並執行生長激素的促生長作用,在細胞層次裡,類胰素生長因子可經自分泌(autocrine)或旁分泌(paracrine)機轉來刺激細胞增生與分化(cell proliferation anddifferentiation)。血液循環中之類胰素生長因子主要是由肝臟製造,其合成與分泌受生長激素嚴密地控制。在循環中,類胰素生長因子被特殊之結合蛋白攜帶著,這類結合蛋白之生理作用有延長類胰素生長因子之半衰期,避免其快速地被排出體外,也可加強或抑制類胰素生長因子之作用。目前,在人類已有六種類胰素生長因子結合蛋白(IGFBPs)被分離出來,其中第一、二、三型之生理作用被研究較多。正常懷孕之胎盤形成時,血管內滋養層細胞(intravascular trophoblasticinfiltration)浸潤至子宮肌層之螺旋動脈(spiral arteires)約發生於妊娠第8週至第18週),但在子癇前症(pre-eclampsia)與原發性子宮內胎兒生長遲滯(idiopathicIUGR)之病例中卻無法觀察到這退行性之血管內浸潤。第一型類胰素生長因子(IGF-I)可能與此退行性之血管內浸潤有關;相對的,在這時期子宮內膜所分泌之第一型類胰素生長因子結合蛋白(IGFBP-1)可能抑制第一型類胰素生長因子(IGF-I)的作用,使子宮內膜免於被滋養層細胞侵犯。因此,提出一個假說:『第一型類胰素生長因子結合蛋白(IGFBP-1) 抑制滋養層細胞退行性之血管內浸潤可能誘發子癇前症(pre-eclampsia)與原發性子宮內胎兒生長遲滯(idiopathic IUGR)』。為了要進一步證實這個假說,本計劃擬將(1)以immunohistochmistry技術來探討正常孕婦及合併有子癇前症與原發性子宮內胎兒生長遲滯孕婦之胎盤內IGFs與IGFBPs分佈之情形;(2)以competitive inhibition及Scatchard分析法來探討胎盤內IGFreceptors分佈之情形;(3)利用reverse transcription-polymerase chain reaction(RT-PCR) 及Southern blotting來探討胎盤內IGFs與IGFBPs之mRNA基因表現之情形;(4)以動物實驗來證實『第一型類胰素生長因子結合蛋白(IGFBP-1)抑制滋養層細胞退行性之血管內浸潤可能誘發子癇前症(pre-eclampsia)與原發性子宮內胎兒生長遲滯(idiopathic IUGR)』之假說。
