Abstract
一位患精神分裂症及癲?酗?12歲女孩，長期使用抗精神病藥物及carbamazepine（CBZ）治療。因懷疑CBZ引起白血球過低而停用，改予valproate控制癲?釩?3天，個案出現精神病症狀加劇及癲?陬o作頻率增加。於提高抗精神病藥物後精神病症狀雖改善，但癲?陬o作之情況仍然持續。其後予以換回CBZ治療後，患者之癲?陬o作狀況又明顯改善。本病例在停用CBZ後導致精神狀況惡化，其可能機制包括：（1）CBZ戒斷之效應；（2）停用CBZ導致癲?陰惆謅ㄗ}所造成之精神病症狀加劇。
A 12-year-old girl with schizophrenia and epilepsy was treated with antipsychotics and carbamazepine (CBZ). Although her psychotic and neurological conditions could be well controlled by these drugs, CBZ was replaced with valproate because CBZ-induced leukocytopenia was suspected. After theprescription change, exacerbation of psychosis and increased seizure frequencywere noted. After increasing the dosage of antipsychotics, her psychiatric symptoms improved. However, she continued to experience frequent seizure attacks until CBZ was administered again. From this case, we have shown that discontinuation of CBZ may lead to the exacerbation of psychosis in schizophrenic patients. The exacerbation of psychosis may be due to CBZ withdrawal or poor control of seizures.
