A chronic patient readmission suggestion system is used for a medical facility to track a postoperative chronic patient and comprises a physiological signal measurement unit installed on a postoperative chronic patient and a back-to-hospital notification unit installed in the medical facility and in on-line connection with the physiological signal measurement unit. The physiological signal measurement unit comprises a measurement module that measures at least a physiological signal and a chronic disease recurrence sign detection module that receives the physiological signal supplied from the measurement module and conducts monitoring. The back-to-hospital notification unit comprises a back-to-hospital suggestion module that is in on-line connection with the physiological signal measurement unit and receives monitoring data from the chronic disease recurrence sign detection module. The back-to-hospital suggestion module also conducts comparison to determine if an abnormality signal occurs, and after occurrence of the abnormality signal, reminds the postoperative chronic patient to return to the hospital for inspection.
