Abstract
本研究目的在瞭解門診躁鬱病患的個人屬性、健康控制信念特質、鋰鹽治療態度
與鋰鹽遵從行為的現況;並分析影響遵從行為之重要變項。 本研究採立意取樣,自民國 84
年 12 月至次年 3 月期間, 針對北部地區某精神專科醫院門診中服用鋰鹽一年以上之躁鬱
病患進行結構性會談,共收集有效樣本 100 位。 將所得資料採描述性、卡方檢定、t 檢定
、皮爾森積差相關及逐步迴歸等分析。個案中只有 41 人有完全符合三項遵從標準(病人自
評、病歷回顧、血中鋰鹽濃度)之遵從行為。性別、每日服用鋰鹽次數、社經階層與鋰鹽遵
從行為呈顯著相關。「有力人士健康控制信念」、「預備行動」態度是影響個案鋰鹽遵從行
為重要變項。「有力人士健康控制信念」與鋰鹽遵從行為呈顯著負相關。鋰鹽治療態度的「
預備行動」與鋰鹽遵從行為呈顯著正相關。根據研究結果有護理實務、行政、教育、未來研
究等方面之建議。
This study aims to examine the relationships among personal attributes, locus of control, lithium attitudes, and lithium compliance in manic-depressive psychotic (MDP) outpatients. Subjects were selected by purposive sampling from the MDP outpatients who were treatedwith lithium for more than one year at a psychiatric hospital in northern Taiwan. One hundred effective questionnaires were completed through structured interview with the sample. Data were analyzed by using descriptive statistics, chi-square, t-test, Pearson correlation, and stepwise multiple regression. Of all cases only 41 patients conformed to all three standards of compliance (i.e., patient self-report, chart review, and lithium serum level) . Gender, daily frequency of lithium intake, and socioeconomic status are significantly correlated with lithium compliance. In addition, powerful locus of control and behavior intention are major predictors of lithium compliance. Higher level powerful locus of control is negatively related to lithium compliance, whereas the behavior intention in lithium compliance are positively correlative with compliance based on chart review. The findings have important implications for nursing practice, administration, education, and research.
