Abstract
Hypercalcemia following renal transplantation is not uncommon, but often resolves within one year. However, when there is persistent hypercalcemia, tertiary hyperparathryoidism should be suspected. The incidence of tertiary hyperparathyroidism falls between 1 and 7 per cent. One patient had experienced acute pancreatitis which resulted from tertiary hyperparathyroidism two months following renal transplantation. Total parathyroidectomy with autotransplantation was performed. The patients clinical course is reported and literature reviewed.
