Screening gallstone disease by ultrasound decreases the necessity of cholecystectomy

Abstract

The natural history of gallstone disease plays important roles in the evaluation of screening. Therefore, the natural history of gallstone disease from no gallstone disease to cholecytectomy were quantified. The initial study cohort was retrospectively conducted with a total of 2386 healthy adults voluntarily admitted to a teaching hospital for a physical check-up in 2002 in Taiwan. Annual follow-up screenings of gallstone disease then were until 31 December 2007. The multistate Markov model was used to assess the natural history of gallstone disease among study subjects. One thousand three hundred seventy-nine subjects who attended at least two sonographic check-ups were studied. The overall response rate was about 57.8%. The mean duration in the disease states single stone and multiple stones were 8.77 [95% confidence interval (CI): 5.78 similar to 66.67] and 6.76 (95% CI: 4.13 similar to 9.90) years, respectively. Compared with the control, the incidence of cholecystectomy reduction for annual, biennial, 3-, 4- and 5-year screenings of gallstone disease were approximately 82.9% (95% Cl: 75.7 similar to 90.4%), 71.6% (95% Cl: 57.0 similar to 88.8%), 64.8% (95% Cl: 46.1 similar to 81.5%), 49.6% (95% Cl: 23.9 similar to 75.3%), and 32.1% (95% Cl: -2.8 similar to 66.7%), respectively. In conclusion, the average time for the development of gallstone disease from nonexistence to cholecystectomy was approximately 33.7 years and annual screening interval of gallstone disease should be recommended.
