Treatment of severely septic patients with fibrinectin-rich cryoprecipitate: A randomised trial

Abstract

Objective: To find out whether treatment with fibronectin-rich cryoprecipitate improved the sepsis severity score in severely septic patients. Design: Randomised, prospective study. Setting: Department of Surgery, Chang Gung Memorial Hospital, Taipei. Subjects: 34 patients with moderate to severe sepsis who had been admitted to the surgical intensive care unit. Interventions: Patients were subdivided according to their pretreatment fibronectin concentration (< 0.1, 0.1-0.15, and > 0.15 g/l, respectively) and each subgroup was further randomised to receive fibronectin-rich cryoprecipitate 20 U. Main outcome measures: Clinical improvement, and reduction in the sepsis severity score. Results: 14 of 16 patients who received cryoprecipitate improved compared with 7 of 18 who did not (95% confidence interval of difference is 21 to 76%). The sepsis severity score was significantly reduced in patients who received cryoprecipitate. Conclusion: There is an association between treatment with fibronectin-rich plasma and reduction in the sepsis severity score in the septic patients.
