罹患急性白血病兒童父母決定中斷治療及再度接受治療之心理過程探討

　　　　　本個案研究為探討一位罹患急性白血病兒童父母決定中斷治療及再度接受治療心 理過程，資料收集的方法包括與病童主要照顧者之正式探入訪談、非正式訪談、參與觀察、 病歷參考、護士之護理報告及資料收集員的反應性日記。由研究結果得知病童父母在中斷治 療及再度接受治療時的心理過程包括：內心掙扎及想要使情況好轉、尋找比較容易接受的解 釋、貶值自我及引發過度保護行為，因此照顧重點除了照顧病童的護理需求外，醫護人員肯 定父母為小孩所做的努力，並隨時給予情緒的關懷及支持，以家庭為中心的護理理念，提供 相互參與的護理模式，將父母視照護上的夥伴，讓父母有醫護團隊和他們在一起奮鬥的感覺 ，朝向提升病童家庭的生活品質的努力。
　　　　　This case study attempts to explore the psychosocial process of parent- al decision on continuation or withdrawal from chemotherapy for actute lymphobl- astic leukemia patients with a 4 years child.Data were collected from interviews with the parents of patient,prospective follow-up,medical charts,and notes of attending nurses.Factors related to the parents' decisions include initial hesi- tation for participating chemotherapy and desire for better treatment,searching an easier and acceptable explanation of diagnosis,self-devaluation,and induced overprotective behavior.Emotional and psychosocial supports for patients;parents are necessary priior to and during the essential for establishing mutual trustf- ul relationship between parents and attending nurses,and thus for the completion of a therapy.
