運用Moos及Schaefer之模式協助一位老人調適遷居至療養機構的護理經驗

　　　　　遷居對老年人而言，是相當大的一個人生事件或危機，如何協助自願或非自願遷 居至安療養機構的老人適應機構生活，將是安療養機構中以護理人員為首的專業中相當重要 的課題，而護理人員更應重視這個問題。首先應從了解老人遷居後的需求和感受為起點，再 言如何協助其適應；此乃本個案研究欲探討之重點。採個案研究法，將實際護理過程寫成 6 篇的行為過程記錄， 探討一位老人面對遷居事件的反應和想法。 參考 Moos 及 Schaefer (1986) 提出的概念架構後，提出本個案研究的概念架構，包括三個主要因素類別， 協助整 理及呈現所得的結果。並提出護理人員如何在機構政策和個案需求的衝突中發揮護理功能， 以及協助老人和照顧者互相接納彼此想法，達到兩者可以接受的程度，使個案較能安全且適 應的度過機構生活的人生階段。
　　　　　The purpose of this case study was to explore the perceptions、needs, and adaptative behaviors of an elderly person who was relocated to a nursing home four months ago. Data were collected through participant observations and interviews during one month period while the researcher was the elder's primary nurse. Content analysis with modificatory Moos & Schaefer's model(1986) was used to conceptionlize the raw data. The elderly person's perceptions、 needs, and adaptative behaviors about relocation were identified. The findings of this study can help nurses to take care of elderly persons who are experiencing relocation.
