Thinned Anterolateral Thigh Flap Used in Burn Reconstruction

摘要

當組織的缺損需要用薄的皮瓣來重建的時候,對整形外科而言是一項挑戰。前外側股皮瓣可提供豐富且可靠的組織做為重建。然而用於在手、足及頸部上,仍顯得過於肥厚。幸好我們可將皮瓣打薄,僅保留真皮下血管網及皮瓣穿透枝附近少量軟組織。這皮瓣可打薄至5-7公厘厚度。本文將討論打薄皮瓣的方法及應用在9位病患的經驗。所有皮瓣均存活,沒有部份壞死的現象。
Defects requiring thin skin flaps present a problem to the reconstructive surgeon. In the past, thicker flaps were used with suboptimal results. The free anterolateral thigh (ALT)flap can provide a large and reliable skin flap for reconstruction. However, it may be too bulky for reconstruction of defects on the hand, foot or neck. Fortunately, a safe and reliable method of thinning ALT flaps has been advocated. By preserving the subdermal plexus and a cuff of tissue around the perforator, the ALT flap may be thinned to5-7mm. We present our thinning technique and which was applied to9cases using a thinned ALT free flap. All cases were effectively reconstructed with complete survival of the flaps.
